So my backplate seems like its fried.
The sync port and the mic port no longer work and there is a crazy sound coming out of the headphone output ( and it amplifies when you put headphones on )
The on screen power indicator says UNKWN as well.

Power chain:
RED BRICK -> ET W-V Mount Battery Module -> RED BackPlate -> Weapon MG

The last thing I remembered doing was using the Power Tap on the ET Battery Module to power a Lectrosonics LR Receiver using a battery eliminator.
After that the plate no longer recognized the Sync port or the mic input. I put our spare backplate on and everything was working. I decided not to use the LR Receiver since I didnt want the same thing to happen.

Any ideas on what may have happened here?

Without having any of the components in front of me, I'd suspect you fried the boards due to the P-Tap being plugged in backwards and/or it's polarity was not correct. Start with checkhing the cable and see what sort of shape it's in.

Thanks Phil. These are the power tap cables we bought.

My sound guy seemed to think it was a polarity issue. If it is in fact a polarity issue, what do you think my next step would be to resolve the issue aside from sending the backplate in for repair?

Since it has a genuine AB PowerTap connector, I highly doubt you managed to insert it backwards...

if the cable had a knockoff PTAP maybe...but with a genuine it's basically impossible unless you push REALLY, REALLY hard.

Well turns out it is caused by pulling out the barrel end of the cable from the receiver while the camera is on. Looking for any kind of explanation.
As I was pulling the rig off my shoulder, the end of the cable was snagged and pulled out from the Lectrosonics Reciever and the camera just immediately shut off. So weird. And turning the camera back on it displays UNKWN on the battery indicator and doesn't receive sound input. Second plate now fried.

Now we know the cause. The next question is what we need to get to secure the barrel end into the Lectro Unit. Its got some threads at the tip of the input but the AB power tap cable we have doesnt have any kind of threads.

That is a weird one! That kind of concentric plug shouldn't allow any kind of short circuit when pulled out, unless the Lectrosonic port has an unusual design.
