Older Sachtler heads do use the step system. Newer Speed Balance (SB) heads from Sachtler are continuous.
Step is a pain. Once you go continuous you'll never be happy with a step system!
No they don't, they just have more steps than they used to.

The Sachtler Video 25 has 18 steps. It's hardly possible to load the camera in such a way that one doesn't get it balanced sufficiently.
Every system has it's pros and cons. The bigger OCons are superb in regards of balance, smooth damping and sheer payload but they are heavy and bulky. The Sachtler Video 25 on the other hand is able to carry over 60 lbs but only weights with CF sticks rougly 22 lbs.
For cine-style shootings I mostly choose a OCon 2060 or the bigger brother, for doc-style shootings a Sachtler Video 20 is fine. The best hybrid for both applications is the Sachtler Video 25 with double retractable CF sticks. It is light enough for docs and has a payload that excepts big cine zooms. Ocon and Sachtler come from the same mother company.
Miller and Cartoni are here in Germany rarely seeded. So unfortunately I haven't used them yet. But by just reading the specs the payload to weight ratio of the Sachtler Video 25 (what a cheap name) is unbeaten.
