How are these catch lights positioned?

I have noticed this style in many Bollywood films. It looks like the actress is crying, but I believe it is just catch lights that I think would be almost straight above the talent so the reflection is off the lower eye and even waterline (avoiding the iris and pupil).

Does anyone know exactly how this look is achieved?

I don't know how it is done, but it is an interesting look to be sure.

If it was directly above the talent, there'd be a pretty big nose shadow.

I suspect they are tearing up the actors with an eye irritant so the eyes water. Some actors can do this through performance but once the eye fills with water it sits at the bottom of the eye reflecting the key.

I think it's also a top light. (on the last 2 frames)
On the other pictures we don't see the shadow coming from a light under their faces. Or is it due to its big size and close to the lens position?
