I had a hell of a time figuring this one out myself, so I thought I'd share my experiences. Here are my system specs. I'm running OSX 10.6.7 and Resolve 8.1 was running fine with my GTX 280 and Extreme 2, but when I updated to 8.2 beta the card would only output a green or black screen to my JVC VT20L1 via Y'PbPr and Samsung LCD TV via HDMI.
I also noticed that the Decklink video output would not "stick" to 59.94 when importing 24p footage and would switch to 1920x1080 24PsF by itself.
Aside from that, I thought maybe the card needed to be re-seated, so I pulled it and restarted- then shut down and plugged it back in...still nothing.
Fearing a bad card, I switched the 280 and the Decklink slots and I was surprised to see that OSX recognized the Decklink as a GPU and was sending my desktop out via HDMI to my LCD. The card obviously was not the culprit.
I deleted the Blackmagic Design Resolve plist file in my Library thinking something got corrupted during the upgrade. Shut down, re-start without the card. Shut down again, switch the GPU and Decklink back to their original slots, re-start and...still nothing. I went to bed pissed off.
To solve it, I did a complete uninstall of Blackmagic Design Desktop Video 8.6 and Resolve 8.2 beta as outlined in this post and rolled back to Desktop Video 8.2.
Another re-start and a re-install of Resolve 8.1 and a second re-start and I'm back up and running.
What's funny is that Resolve 8.1 updated my Desktop Video to 8.6 and it still works, so 8.2 looks like the culprit here. Beware! At the moment it appears Resolve 8.2 beta does not support HD Extreme 2 cards in OSX 10.6.7
