Proxys not made what may have happened ?

What causes for proxys not to be made?

There are some loads, and some takes that have proxys missing.

I know i can make them and i can use the R3D to make QT files.

But I was blamed for the issue on a shoot where I was the D.I.T.

I didnt erase them so I want to answer them with possibilities.

Did the "make Quick Times" setting get disable in the camera somehow?

I was on build 16,
The make Quick Times settings did not get disabled somehow.
It happened only on a few shots very un predictable.

Im still clueless to why this may have happened.
