Most helpful slate info and notes

I am shooting a commercial and handing footage off to an editor and post house. I typically handle our own in house editing. What information should I include on the slate and what kind of notes if any would be expected from me?
There will be one tracking shot (replacement of a computer screen). Other than that a fairly typical shoot.

Post guys - what info do you wish you had from a DP?

Definitely include focal length of lens - especially for the tracking shot.
