LMB-5 3-Stage Back?

Anyone know who makes a 3-stage back for a LMB-5?

I've seen these, but I can't remember who makes them.

Wider lenses many vignette with a third stage installed on an LMB-5. For wider coverage, the LMB-15 has a wider angle hood in the front and a larger clamp ring option on the back. The LMB-15 is available in two or three stage variations. We use the 3-stage LMB-15 on an Optimo 15-40 for Super-35 coverage with no issues.

I have one of my LMB-5's set up with the Lentequip 3-stage deal. Very nice.
I also did one of my LMB-4's this way too.
You can go out to a 14mm S4 with no problems.
You will have to call Arri and order another tray.

Now just have to work out a way to get a 134mm back for a LMB-15.
