I think one problem is that the sound guy rarely feels empowered to speak up. He essentially keeps his mouth shut, holds his bag and boom and hopes the lavs can save the sound. If he starts interjecting between takes, he's going to be seen as a trouble maker, so he doesn't. Hence he works under conditions that are less than ideal to get good sound and, as a consequence, a lot of it has to be ADR'd.
