Hi everyone, this one has had me puzzled for a very long time. I acquired this in a lot of camera gear over a decade ago. I only have shot 35mm Digital DSLR's. I do know that the front of the lens-the protruding front element, fits into a long Canon FD mount lens.
The other side that mounts to a camera is not a mount I am at all familiar with. My only guess is a OpTex Universal mount that take another adapter to go to a cinema/movie camera mount?

Amy thoughts greatly appreciated-looks like high quality glass

I thought it might be a CP-Mount or Aaton mount at first glance but it looks a bit larger. The CP16/CP-GSMO and Aaton cameras were 16mm/Super16mm motion picture film cameras. There was an adaptor for Nikon to CP-Mount which used a planar glass rear element for optical correction.

The Canon FD mount was common when these motion picture cameras were being used so it is logical that Optex, a UK outfit, may have made an adaptor for the european market, maybe for the Aaton. Please do not take notice of my comment. I am really only guessing. The CP and Aaton mounts were similar and may have even had the same flange to focal plane distance but are not identical or interchangeable as far as I know.

There are CP to E-mount, M43, EOS mount adaptors for the CP Ultra T* lenses but I think there may be direct adaptors for the FD-Mount lenses which may be less expensive. Search eBay under Cinema Products and you will find them.

Thank you Robert. That gives me a place to start research. I was curiously trying to find out what this "OpTex Universal mount" was, and if perhaps this item was sort of like the Tamron adaptall lens series was-where you add an adapter of choice to the universal bayonet on rear of lens mount.

Being that this adapter was very narrow, it seemed "perfect" that is was a universal "stub" that an adapter of choice could be added to fill out the unit for performance.

In a fast test, I taped the converter to my m4/3 body that has an Olympus OM-m4/3 adapter on it and added a Legacy Zuiko 300/4.5 manual lens, and it was just about right for infinity focus and closer..so it is darn close for 35mm film work, but the Canon EOS is the only mount that is about right diameter wise..so I am still thinking it is cinema/or universal in need of adapter.

Again, thank you for your thoughts, very much appreciated!

John
