I was on a shoot and in a hurry and went to black shade (but totally forgot to take off lens and with cover on ef mount) totally realized it but was too late started black shading with lens on without cap. Did use my eye brow on my matte box in about 5 secs and just let it do its thing (didn't turn it off). Got a error message and said to return to RED. Turned off camera and turned it back on and did a proper black shade and said it was successful, is my sensor/camera okay (didn't act up or anything) Has this every happened to anyone and is my Dragon okay?

Black shade doesn't affect the sensor. You are only telling it that what it is seeing is truly black, so that it can subtract fixed pattern noise, sensor shading, and bad pixels from recorded images.
In a pinch you can close your iris right down, put a lens cap on and it should still black shade fine. The only caveat is some plastic lens caps might not be opaque to infrared light, which is why I would cover the lens from any strong light too if I was blackshading with the lens still attached.

Your first attempt failed because it detected that there were some light leaks.

Your Dragon is fine.
